We work with Medicare and most major insurance plans. Have questions about your coverage? Call us — we'll help you figure it out before your first visit.
Call (828) 696-9411We are in-network with the following plans. If you don't see yours listed, call us — we may still be able to see you as an out-of-network provider.
Insurance participation can change. Please call to confirm your specific plan before your visit.
We are a Medicare-participating provider. Medicare Part B covers outpatient physical therapy when it is medically necessary. There is no annual dollar cap on Medicare PT benefits — coverage continues as long as your treatment is medically necessary and you are making progress.
Medicare typically covers 80% of the approved amount after your Part B deductible is met. If you have a Medicare supplement (Medigap) plan, it may cover the remaining 20%.
Medicare does require a physician referral (from an MD, DO, NP, or PA) before starting physical therapy. Medicare Advantage (replacement) plans vary — many do not require a referral, but check your specific plan. For all other insurance and self-pay patients, North Carolina is a direct access state and no referral is needed.
Many patients choose to pay out-of-pocket — either because they prefer not to use insurance, their plan has a high deductible, or they want care that isn't dictated by insurance company rules. We welcome self-pay patients and are happy to discuss our rates when you call.
If you have a PPO plan, you may have out-of-network benefits that reimburse a portion of your PT costs even if we're not in your network. We can provide a superbill (itemized receipt) that you submit to your insurance for reimbursement.
Call your insurance company and ask: "What are my out-of-network outpatient physical therapy benefits?"
It depends on your insurance. North Carolina is a direct access state, so most commercial insurance and self-pay patients do not need a referral. However, Medicare requires a physician referral (from an MD, DO, NP, or PA) before starting PT. Medicare Advantage plans vary — many don't require one, but check your specific plan. When in doubt, call us and we'll help you figure it out.
Call the member services number on the back of your insurance card and ask: "Is Flat Rock Physical Therapy for Pain in Hendersonville, NC in my network?" You can also call us at (828) 696-9411 and we'll help you verify.
This depends on your specific plan. Most insurance plans have either a flat copay per visit (often $20–$60) or a coinsurance percentage (often 20–30% of the allowed amount) after your deductible is met. Your insurance card or member portal will show your PT benefits.
Some plans do limit visits per year. Medicare does not have a visit cap, but does require that treatment remain medically necessary. If you're approaching a visit limit, we'll let you know so you can plan accordingly.
We accept cash, check, and all major credit and debit cards. Payment is due at the time of service for self-pay patients and for any copays or coinsurance amounts.
